The R.A.I.N.Case Management Program serves home bound seniors 60 years of age and over residing in Bronx Community Districts 7,8,11, and 12. Case Managers assess, coordinate, monitor, evaluate, and advocate for multiple services and interventions to meet the needs of the seniors.
Our case managers are trained professionals who assess and link seniors to a wide range of services such as transportation, housekeeping, home care, home delivered meals and assistance with entitlements and benefits. The program strives to provide excellent services with the goal of helping seniors to continue to live safely in their homes and to re-engage in their communities. We strive towards the integration of care delivery.
Our Friendly Visiting Program pairs older adults with volunteers for in-home visits. Volunteers visit on a regular basis to talk about shared interests and experiences in the process. Volunteers may also take walks with older adults and help run errands. All volunteers will be screened and trained.
If you or someone you know would like to make a difference in someone's life as a Friendly Visitor, please contact us at 718-231-7246.
